- Death Letter/ Divorce Certificate
- The Letter of Citizenship ( surat WNI )
- The Letter of Change Name ( akte ganti nama )
- Reference Letter from The Company ( surat referensi )


PASSPORT CONDITIONS FOR INDONESIAN

- Previous expired passport ( pasport lama )
- Identity Card ( KTP )
- Family Card ( KSK )
- Photograph 4 x 6 = 2 pc
- Birth Certificate ( akte lahir )
- Certificate of Elementary Education
- Marriage Certificate ( akte nikah )
- Certificate of Criminal Conviction ( SKKB dari POLRES )
- Reference Letter from The Company ( surat referensi )
- LITSUS ( Special for Government Officer )


Another than above conditions, for children who wants to get a passport should be enclosed a TEMPORARY IDENTITY CARD FROM THE SUBDISTRICT.
